Complete an application form and fax it to (435) 882-4000. We will review it and get back to you as quickly as possible. Sometimes faxes don't go through. If you haven't heard right back from us, please give us a call to make sure your application arrived.

HOW DO I GET SOMETHING REPAIRED?

Please submit all maintenance requests by email. We track all maintenance requests by email. The best way to get your maintenance on the schedule is to email it. Please call only in the case of an emergency. Thank you!
